Osteoporosis Care After Hip Fracture: A Regional Healthcare System Experience

Abstract Background: Pharmacologic treatment is recommended to reduce risk of future fractures and possibly mortality in patients with hip fracture. We investigated osteoporosis care after fracture at a regional comprehensive healthcare system identify rates pharmacologic barriers treatment. Methods: identified all admitted low impact between 1/2017-12/2018. Follow-up clinical data was collected for minimum 16 months Results: 208 were fractures: 130 (62%) female, mean age 79.6 (SD 12.6), 24 (12%) nursing home resident, 117 (56%) had BMI <25 kg/m2. At the time fracture, 80% polypharmacy, 42% used mobility aide, 24% known osteoporosis, 22% dementia/cognitive impairment, 20% has history cancer, stroke, 19% diabetes 2% on dialysis. Two hundred (96%) underwent surgery. Forty-three (20%) vitamin D level checked, this, 20 (46%) <30 ng/mL. Prior admission prescription 53% calcium 36%. Discharge 64% 50%. 18/208 (9%) prescribed medication 1 year following 26/192 (14%) (11 new, 15 continuation medication). For follow up, 114/192 (59%) seen orthopedics clinic, 61 (32%) primary 2 (1%) endocrinology clinic 99 (52%) other clinics. Sixteen (8%) died during hospitalization 47 (22%) within year. Conclusions: Osteoporosis suboptimal model needed close this gap.
